A prospective multicenter study published in the Circulation journal assessed the performance of high-sensitivity cardiac troponin I in patients with suspected acute coronary syndrome, both with and without renal impairment (estimated glomerular filtration rate < 60mL/min/1.73m2). It was reported that high-sensitivity cardiac troponin identified lesser patients with renal impairment as low risk and more as high risk, but with lower specificity for type 1 myocardial infarction. Moreover, patients with renal impairment and high cardiac troponin concentrations were at a 2-fold greater risk of experiencing a major cardiac event than those with normal renal function, regardless of the diagnosis.
